Civic "Octoberfest" Dinner

Meet & Greet 5pm with dinner at 5:30pm. 

This is the one and only official RSVP Cancellation form for the Civic dinner reservations and is required for official cancellations. Please understand, we are contracted to pay the 19th Hole based on the number of dinner reservations filed by the RSVP deadline, regardless if you actually attend or not. Please report a cancellation as soon as you can. Cancellations after the RSVP deadline can be reported and we will do our best to help fill it but it is ultimately your responsibility to honor that commitment and dinner cost.

Please make sure to complete all prior reservation data as it applies to each individual's cancellation. We will only cancel the individual members/guests you note. When complete, click the "Cancel RSVP" button. 

If you do not see the steps outlined, attempt to refile. Please note that there are a few required fields. If a required field is not answered it will not complete the form. Required fields are marked with a red circle and/or the word "required" by the field.

RSVP Cancellation Process:

  • Please answer the questions below as they apply to you to enable the Civic Association to accurately cancel your dinner reservation. 
  • Upon completion of entering your information, click the "Cancel RSVP" button at the bottom of the form. 
  • A confirmation message will appear on the follow-up page: 
    • This is your official confirmation notice!
      • You have reported your cancellation for the event and the Civic Association has received it.
      • If you want a record for your own documentation, please print the page. 
    • No other confirmation notices will be sent.

More than 7 members + guests in a your party needing to cancel reservation, please submit two forms.

CANCELLATION

  • Select "Cancel my entire reservation" if needing to cancel the complete reservation originally submitted, and then, complete Member 1 information along with a "Not Eating" dining preference.
  • Select "Cancel part of my reservation" if needing to cancel an individual(s) from the originally submitted reservation, and then, add all members and guests that are coming and leave the non-attendees off.

BEWARE, USING THE AUTOFILL FEATURE ON YOUR DEVICE WILL REPEAT THE FILL TO ALL THE SLOTS!
